Don't debride all "heel caps".

Heel ulcers with dry eschar do not generally need debridement unless they have erythema, fluctuance, or drainage.

Patients with poor arterial flow are at high risk of infection once wound is open.

They should be monitored daily for any complications that would require debridement and removal of the dry "cap."

Many will resolve on their own without intervention.

Once debrided, there is an open wound that will take months to resolve.
  • Monitor heel ulcers daily.


  • Defer debridement if possible.
