Order a home assessment to check the environmental hazards.

Environmental factors like loose rugs, cords, or clutter in walking paths, poor lighting, or tall shelves that require precarious behavior, can increase the risk of falling.

When you discharge an older patient at risk of falling, order a home assessment by an occupational therapist (OT) for assessment and modification of environmental hazards.

The OT can recommend safety equipment, like grab bars and raised toilet seat for patients with difficulty standing up.
