Don't wait for Neurology, Psychiatry, or Geriatrics consult before taking basic first steps

Often, when older patients become acutely confused, consultations are requested from neurology or psychiatry.

This is entirely appropriate.

However, the basic steps in the "DO'S" section of this chapter are easily performed and can be done before consults arrive, saving time.

Time is an important consideration; much can be lost by delays.

Depending on the institution and the day, 6-48 hours may elapse from the time the consult is first requested to when the recommendations are implemented.

Delirium is an acute process that may progress and evolve in this time span.

If it is related to medications, the delirium may be aggravated by continuing those medications, and chemical abnormalities of the blood may also worsen.
  • Have basic data available for consultants to review, such as:

    - blood glucose levels

    - pulse oximetry

    - urinalysis
