Don't arrange PEG placement in a diabetic or someone with symptoms of impaired gastric emptying without ordering a gastric emptying study first.

Symptoms of delayed gastric emptying may include early satiety, nausea and vomiting after food.

Feeding through a gastrostomy tube in someone with delayed gastric emptying will result in gastric distention, discomfort, vomiting and a high risk of aspiration.

Such patients require the feeding tube to be positioned in the distal duodenum beyond the Ligament of Treitz (PEG-J).

It is important to order an isotope gastric emptying study in all diabetics and anyone else with suggestive symptoms prior to arranging PEG placement.
