Ask for help from healthcare providers familiar with the patient or from the Geriatric Consult Service.

Knowledge Action
In most cases, you will not be familiar with the patient before they are admitted to your service and it may be difficult to view their current illness in the context of their long-term disability.

It can be difficult to know what is in the patient's best interests and it may help to involve a physician, nurse practitioner or other professional who knows the patient in the decision-making process.

This is a tough decision for the family and the input of a healthcare provider who has an established relationship with them can be invaluable.

Even if you feel comfortable handling the decision-making process yourself it is courteous to inform the patient's primary care physician.

The Geriatric Consult Service is staffed by physicians who have considerable experience in helping patients and families with end-of-life decisions and would be happy to help you with counseling families or suggesting other ways to manage the patient.
